Community Care contracts with United Community Center

MILWAUKEE – Two local nonprofits are teaming up to serve seniors and adults with disabilities in Milwaukee County.

Leaders from Community Care, Inc. (CCI) and the United Community Center (UCC) finalized the details of a plan earlier this month to start Aug. 1, 2022.

Under the arrangement, UCC staff will provide care management services to CCI members enrolled in the state’s Family Care program, which provides long-term care to seniors and adults with disabilities. Many of the members served through this arrangement with UCC will be Spanish speaking.

Both organizations provide a variety of services to underserved communities in the Milwaukee area. UCC has provided contracted care management services through the state’s Family Care program in Milwaukee County for nearly 20 years.

“This plan not only strengthens our relationship with a trusted community partner, but adds to Community Care’s ability to serve our diverse community,” said Kenneth Munson, Community Care’s chief executive officer. “Their knowledge and experience will be an asset to Community Care.”

In 2003, UCC began serving frail elders in Milwaukee County who were Medicaid eligible and enrolled in the Family Care program.

During the last nearly two decades, UCC has expanded its scope of service to include adults with physical, developmental, and/or intellectual disabilities, becoming the primary agency providing managed care services to the Hispanic community in Southeastern Wisconsin.

“Our partnership with Community Care, Inc. will allow UCC to bring its decades of expertise and industry leading care for the Hispanic community and blend it with the innovative, and flexible community-based program of Community Care,” said UCC Executive Director, Laura Gutierrez. “This will allow us to optimize the quality of life of those we serve and empower them to thrive and achieve their fullest potential.”
