Sixteenth Street Community Health Centers Ranks Among Top 10% Nationwide, Only Wisconsin Health Center to Earn Gold Quality Award
Milwaukee health center earns Gold Health Center Quality Leader recognition and additional honors for 2025 performance
MILWAUKEE — August 2026 — Sixteenth Street Community Health Centers is proud to announce that it has been recognized among the top 10% of health centers nationwide for its commitment to providing high-quality care to the Milwaukee community. Sixteenth Street is the only health center in Wisconsin to receive this national Gold-level distinction for 2025 performance.
Sixteenth Street received the 2025 Gold Health Center Quality Leader Award from the Health Resources and Services Administration (HRSA), the highest level of quality recognition that a Community Health Center can achieve. The Gold designation recognizes health centers that achieved the best overall clinical quality measure performance among health centers nationwide.
Among nearly 1,400 health centers operating more than 16,200 service sites nationwide, Sixteenth Street earned a place in the top 10% nationally and is the only health center in Wisconsin to receive the 2025 Gold Health Center Quality Leader Award.
In addition to the Gold Health Center Quality Leader Award, Sixteenth Street received two additional HRSA recognitions for 2025 performance, along with a Patient-Centered Medical Home (PCMH) badge, highlighting the organization’s work in health center quality, heart health, and health information technology.
